2.VISA APPLICATION FORM
3.PHOTO (1copy of 4.5cm x 4.5cm with white background.)
4.Daily Schedule (Prepared by the applicant or the taravel agency.)
5.CERTIFICATE of Membership(Representative of the organization should submit copy of Certificate of Business Name Registration issued by DTI. If not yet registered,explanation and proof of existence of the organization are required.)
6.SOCIAL SECURITY CARD COPY
7.TRAVEL ORDER / DISPATCH LETTER from the organization in the philippines
1. INVITATION LETTER
(Required if the Applicant is invited by a Japanese Counterpart)
2.Business registration(TOKI TOHON) / company profile or explanation about the organization.
- Employment Certificate is required if an individual is inviting the applicant.
3.DOCUMENTS THAT CLARIFY THE APPLICANT'S ACTIVITIES IN JAPAN
(Conference agenda, Brochure of organization, News letter./event programme issued by the organization)
4.Guarantee Letter
(If counter part in Japan will shoulder any part of expenses.)

- Validity of documents is three(3)month from the date of issue(unless otherwise stated)
- As a general rule Original Documents will not be returned. If there are original documents that are difficult to replace please enclose a copy so we may return the originals when the Visa is granted.
