Broken lamination of the Picture Portion will not be accepted / Must have at least 2 blank pages left in the passport.

3.PHOTO (1 Copy of 4.5cm x 4.5cm with white background.)

4.BIRTH CERTIFICATE (NSO issued with receipt and valid within (1)year.)

Applicants holding (old or valid) passports with used Japan Visa will be exempted from submitting of birth certificate.

IN CASE OF NON-RECORD OF REGISTRATION OF LIVE BIRTH
Additional Requirements as follows.

Non-Record in the NSO. Need certificate of Non-Record from the NSO with BIRTH CERTIFICATE from the LOCAL CIVIL REGISTRAR.
Non-Record in the NSO and LOCAL CIVIL REGISTRAR. Need certificate of Non-Record from the NSO and the LOCAL CIVIL REGISTRAR.

IF NSO ISSUED RECORD IS NOT CLEAR

BIRTH CERTIFICATE from the LOCAL CIVIL REGISTRAR.

IF BIRTH CERTIFICATE IS LATE-REGISTRATION

Birth Certificate from the LOCAL CIVIL REGISTRAR.
Original Baptismal Certificate, School Record (Form 137 or report card) School Year Book.

5.MARRIAGE CONTRACT (If married) . NSO issued and valid within(1)year.

Applicants holding (old or valid) passports with used Japan Visa will be exempted from submitting of marriage contract.

10.DAILY SCHEDULE IN JAPAN (TAIZAI NITTEIHYO)

1. Invitation letter from guarantor in japan.(Must have handwritten signature.)
(Letter must be more than 200 Jap. characters) Explain clearly reason for visit.

2.DOCUMENTS TO PROVE RELATIONSHIP TO FRIEND, ACQUAINTANCE, FIANCEE
(Please submit as many documents as possible.)

Submit as many Photos together with friend, acquaintance, fiancee
International Phone records (Bills with the applicants Phone number) within past(1) year.
Letters / Bank remittance records / e-mail records (with in past (1) year.)
Invitation Letter from the Parents (Letter must be signed with INKAN attached with certificate of registration for the INKAN from City Hall) valid 3mos.
FAMILY REGISTRATION (KOSEKI TOHON) Valid 3mos. from date of issue. (If inviting fiancee.)

3.RESIDENCE CERTIFICATE (Valid for 3mos.) (jyuminhyo,if guarantor is a japanese.)

Foreign Registration Certificate from city hall.(If guarantor is NOT a japanese.)

4.GUARANTEE LETTER

If guarantor in japan shoulders any part of travel/living expenses.
