• Broken lamination of the Picture Portion will not be accepted / Must have at least 2 blank pages left in the passport.

4.BIRTH CERTIFICATE (NSO issued with receipt and valid within (1)year.)

• IN CASE OF NON-RECORD OF REGISTRATION OF LIVE BIRTH

1. Non-Record in the NSO.Need certificate of Non-Record from the NSO headguarter with birth certificate from the local civil registrar.
2. Non-Record in the NSO and local civil registrar.Need certificate of Non-Record from the NSO headguarter and the local civil registrar.

• IF NSO ISSUED RECORD IS NOT CLEAR

1. BIRTH CERTIFICATE from the LOCAL CIVIL REGISTRAR.

• IF BIRTH CERTIFICATE IS LATE-REGISTRATION

1. Birth Certificate from the LOCAL CIVIL REGISTRAR.
2. Original Baptismal Certificate, School Record (Form 137 or report card) School Year Book (if applicable)
   (Need contact tel # of the church,and contact tel# of the school)

5.MARRIAGE contract (NSO with validity of not more than (1) year from date of issue.)

1. In case of Non-Record in the NSO.Need certificate of Non-Record from the NSO headguarter with Marriage certificate from the local civil registrar.
2. Incase of Non-Record in the NSO and local civil registrar.Need certificate of Non-Record from the NSO headguarter and the local civil registrar.

Applicants holding (old or valid) passports with used Japan Visa will be exempted from submitting No.4 and No.5

1. Letter stating the reason for traveling to japan(With Guarantor�fs Signature.) (Must consist of more than 200 Japanese characters.)

2.FAMILY REGISTRATION (KOSEKI TOHON)Validity is (3)months from date of issue.

3.PASSPORT COPY OF THE JAPANESE SPOUSE (Except blank pages)

4.Photocopy of the ACR I-card of the Japanese spouse.

1. IF THE APPLICANT REQUIRES MULTIPLE ENTRY. Please state the reason for the request in the letter. Also submit a marriage contract or any supporting documents to show that the applicant has been married for more than one year.
